Inappropriate pro/con.

چکیده

I have major concerns after reading the Pro/Con arguments presented by Drs. Phillips and Powell about the role of upper-airway surgery in the treatment of sleep apnea in the July issue of the Journal of Clinical Sleep Medicine 2005;1:236-45. Although I realize that the Pro/Con format encourages expressions of one’s opinions about the literature, these two presentations took the level of unscientific opinion and emotional expression beyond acceptable limits. Dr. Powell’s accusation that the sleep scientific community is overly critical of research on the surgical treatment of obstructive sleep apnea (OSA) is unfounded. Credible reviewers critique articles based on their scientific merit. Sometimes comments are critical because of the lack of adequate controls or randomization in some surgical studies. Sometimes reviews forget that large, randomized, controlled trials cannot be conducted with a surgical procedure, a fact of life with which we all have to live. In his comments about surgery and continuous positive airway pressure (CPAP) being equivalent methods of resolving the airway narrowing, Dr. Powell ignores the role of increased upper-airway compliance or collapsibility, a variable not corrected by surgery, unless there is unintended fibrosis produced by the particular surgical procedure. This involvement of upperairway collapsibility in the mechanism of upper-airway narrowing in sleep invalidates Dr. Powell’s argument that CPAP and upperairway surgery are two equivalent ways to control upper-airway size and, therefore, OSA. To propose that sleep doctors think that “surgery has no place in the treatment of OSAS...” is an overstatement of any objective reader of the literature. Dr. Powell’s statement that surgical investigators have more responsibility than their sleep-medicine investigator colleagues is obviously a biased, inflammatory, and inappropriate stance. Dr. Phillip’s comments are even more disturbing. To criticize OSA surgery as “leaving a deadly level” of OSA postoperatively is a biased unscientific statement. When a CPAP mask is removed in the middle of the night, the patient has persistent apneas and is

Pro-Con Debate: Protracted Bacterial Bronchitis as a Cause of Chronic Cough in Children.

Pro: Children with chronic cough present a diagnostic challenge. Protracted bacterial bronchitis (PBB) is a chronic, persistent bacterial infection of conducting airways defined by the presence of cough for longer than 4 weeks that resolves with antimicrobial therapy and without an alternative diagnosis. Changes in some pro-and anti-inflammatory cytokines produced by bovine peripheral blood mononuclear cells following foot and mouth disease vaccination

Interleukin (IL)-17 is exclusively produced by CD4 helper T-cells upon activation. It most often acts as a pro-inflammatory cytokine, which stimulates the release of pro-inflammatory cytokines IL-6, IL-8, TNF-α, and granulocyte-macrophage colony-stimulating factor (GM-CSF). In this study, we studied the in-vitro IL-17 response to specific antigens and a variety of mitogens and compared the IL-1...

Effect of Dietary Supplementation of Procyanidin on Growth Performance and Immune Response in Pigs

This study was performed to determine the effect of dietary supplementation of procyanidin on growth performance, blood characteristics, and immune function in growing pigs. In experiment 1 (Exp. 1), thirty-two crossbred pigs with an initial BW of 19.2±0.3 kg were allocated into 4 treatments for an 8-wk experiment: i) CON (basal diet), ii) MOS 0.1 (basal diet+0.1% mannanoligosaccharide), iii) P...

Relaunching the Pro–Con section at Critical Care

At a recent Editorial Board meeting of Critical Care that was held in conjunction with the International Symposium of Intensive Care and Emergency Medicine in Brussels, there was a fairly lively discussion about the Pro–Con section of the journal. Despite data demonstrating that the diverse and provocative Pro–Con debates published over the years have been popular, there was a sense they could ...

A New Method for Re-Analyzing Evaluation Bias: Piecewise Growth Curve Modeling Reveals an Asymmetry in the Evaluation of Pro and Con Arguments

In four studies we tested a new methodological approach to the investigation of evaluation bias. The usage of piecewise growth curve modeling allowed for investigation into the impact of people's attitudes on their persuasiveness ratings of pro- and con-arguments, measured over the whole range of the arguments' polarity from an extreme con to an extreme pro position. Moreover, this method provi...
